Office Policies
Due to the high volume of phone calls, return
calls from Dr. Hogan cannot always be made on the
same day. Dr. Hogan seldom has time between
patients to return calls. Calls that need a reply
on the same day will either be returned by Sharon
Jung, ARNP, within the next day or by Dr. Hogan on
Fridays.
If there is a question that can't be answered
quickly, it is best to ask for a short appointment
to discuss the problem. Our neurological nurse
practitioner, Sharon, can usually see you promptly
to give you the care that you need. Extended phone
conferences requiring greater than five minutes
will be charged at a rate of $10.00 per five
minutes. You will receive a separate bill for this.
Insurance does not pay for telephone
conferences.
Medication Refills
If you need a prescription refill, it must be
called into the pharmacy at least 24 to 48 hours
prior to when you need to pick it up. Call the
pharmacy directly to see if your prescription has
been called in. Please do not call the office.
Legal and Insurance Forms
Dr. Hogan will complete all forms that need to
be filled out on Fridays. In most cases, you will
be charged a fee. When you drop the form off,
please check with the receptionist to find out how
much we charge.
Rescheduling and Cancellation of
Appointments

Hogan, we require that you give us 24 hour notice
if you need to reschedule or cancel your
appointment 24. If you miss an appointment without
calling to cancel, we will bill you a fee of $25
for the appointment time. Some situations will be
excused.
Test Results
Test results, including lab work, MRI results,
EEG results, and other testing will be discussed at
your follow-up appointment unless Dr. Hogan needs
for you to be aware of the results sooner.
Medical Records
We will be happy to forward a copy of your
medical records to another physician at no charge.
However, if you want a copy for your own personal
records, you will be billed a copying fee. Please
check with the receptionist to determine the
cost.
Insurance Referrals
If you have an insurance that requires a
referral, it is your responsibility to make sure
you have a valid referral for your appointment with
Dr. Hogan. If Dr. Hogan orders tests that require
referrals, the receptionist will call to initiate
the referral process, but it is ultimately the
responsibility of the patient to check that a
referral has been authorized before any testing is
done.
